A Fifth Avenue plastic surgeon turned a glamorous model into a “sex slave,” subjecting her to sickening abuse — and tried to cover up her black eye by injecting her with filler without anesthetic, an explosive new lawsuit claims.
Ammar Mahmoud, 40, at one point allegedly beat his ex-girlfriend Maya Willow Sias, 25, so savagely he shattered her eye socket, according to the $10 million suit filed Tuesday.
Sias alleged that Mahmoud secretly then took her to his Alinea Medical Spa, where he injected fillers around her still-swollen eye — a tortuous procedure he did with no anesthetic, the suit states.
“He stabbed me in the face with a needle, I could feel the needle scratching against the bone,” Sias told The Post in an exclusive interview.
The Los Angeles-based singer and model, who once appeared in “Maxim,” contends in the Manhattan Supreme Court suit that Mahmoud is a violent drug- and alcohol-abusing sex addict hiding behind the facade of a caring plastic surgeon.
She also believes Mahmoud has allegedly beaten and abused other women — an accusation echoed in an ongoing 2014 lawsuit filed by the doctor’s ex-wife.
Sias documented the bruises and other injuries she allegedly received at Mahmoud’s hands during their nine-month relationship in stomach-turning photos littered throughout the 16-page civil complaint.
“There is no ‘she said’ here,” her attorney Larry Hutcher, a partner at Davidoff Hutcher & Citron LLP, told The Post.
“He literally made her into a sex slave.”
Sias met Mahmoud during an April 2023 visit to Miami, where they both attended a party on a yacht, according to the lawsuit.
She told The Post that Mahmoud tried to get her into a threesome with him and his girlfriend at the time.
The model declined, but said she nonetheless was charmed by Mahmoud and kept in touch.
Shortly afterward, she came to New York City for another modeling job and, after a whirlwind date that included dinner at the fashionable Meatpacking District restaurant Catch, began a relationship with him, the lawsuit states.
Mahmoud allegedly love-bombed Sias with extravagant gifts and trips to Miami and Dubai before he offered to ship her belongings from her North Carolina home to his Fifth Avenue pad, according to the lawsuit.
But Sias soon encountered Mahmoud’s dark side, including his regular abuse of the anesthetic ketamine and 2C, a synthetic psychedelic drug, the suit alleges.
“Almost nightly, Dr. Mahmoud would abuse drugs and alcohol, and force himself upon Willow at all hours of the night, irrespective of whether Willow consented to the sexual acts, or was even conscious,” the lawsuit states.
After a June 7, 2023 dinner with Mahmoud, Sias awoke in the middle of the night to find him allegedly partying with three prostitutes in the apartment, the lawsuit states. She confronted Mahmoud and he flew into a violent rage.
“After the women left, Dr. Mahmoud violently hurled Willow to the ground, mercilessly punched her face and body, and smothered her face with a pillow, causing her to lose consciousness,” the suit states.
“I thought I was going to die,” Sias told The Post.
The savage beating left Sias unrecognizable, with her eye swollen shut, orbital bone shattered, head misshapen and body and face covered in blood and bruises, according to the lawsuit.
Mahmoud apologized, but allegedly forced her to stay inside his apartment so no one could see her injuries, monitoring her location by her phone and threatening severe consequences if she tried to leave, court papers state.
The lawsuit contends during this time that Mahmoud exploited his status as a medical doctor to prescribe Sias antidepressants, in addition to forcing her to take ketamine to “keep her obedient and compliant.”
In late July, after Sias’ bruising and swelling subsided, Mahmoud allegedly whisked her over to his clinic to treat her shattered eye socket — and cover up his abuse, the lawsuit asserts.
“Despite her obvious need for proper medical treatment, Dr. Mahmoud took it upon himself to simply conceal the physical damage he had inflicted, under the guise of his own medical license, which he abused,” the suit states.
Mahmoud took injected fillers to Sias’ eye area without anesthetics, a sadistic act of “calculated cruelty,” according to the lawsuit.
“Willow was left physically and mentally traumatized by the torturous experience,” the lawsuit states.
“The pain from the injection was compounded by the emotional anguish of knowing that her abuser was also her only option for care, who abused his medical authority to further control and harm her,” it reads.
“I don’t get to be like I’m ‘all natural’ now because I have filler in my eye,” Sias lamented to The Post.
The alleged abuse, manipulation and drugging continued for three more months, including an October 2023 incident in which a woman — who was in a guest room with Mahmoud, repeatedly punched Sias in the face as the doctor held her down, court papers claim.
Mahmoud’s alleged sexual abuse escalated as well, “forcing her to participate in non-consensual sexual activities with strangers, sexually assaulting her while she was unconscious, and physically restraining and threatening her,” according to the lawsuit.
“Specifically, Dr. Mahmoud forced Willow to participate in threesomes with prostitutes, even though she expressly told him ‘no,’” the lawsuit states.
Sias said she finally escaped Mahmoud after she went to California for a job and never returned, even leaving all her possessions at his apartment.
The lawsuit — which accuses Mahmoud of violating the city’s victims of gender-motivated violence protection act, intentional infliction of emotional distress and negligence — asserts Sias’ career has suffered because of her emotional and physical injuries.
Sias, who now lives in LA, said she stayed in the horror with Mahmoud for so long out of fear, and that she now largely keeps to herself at home.
“I was scared that he was going to retaliate and kill me,” she said.
The lawsuit states Sias believes Mahmoud allegedly beat and abused other women — and court records show it’s an accusation made before.
Mahmoud’s ex-wife filed a lawsuit, also in Manhattan civil court, that accuses the doctor of sexually assaulting her during their short-lived marriage and forcing her to take a human growth hormone because he did not “approve” of her body.
He also allegedly repeatedly beat her, including one incident in which he smothered her with a pillow, according to the lawsuit.
Mahmoud filed a defamation lawsuit against his ex-wife in 2019, strongly denying the accusations as “outright fabrications.”
His ex-wife couldn’t be reached for comment.
Both cases remain open, court records show.
Mahmoud, when reached by Post, said he “unequivocally” denied the accusations and that they were not reflective of his character. He declined further comment and said he’d be arranging legal representation.
